Job summary

CVS Health is hiring a Pharmacist for our Specialty Mail Service Pharmacy in Chandler, AZ. This full-time role supports patient care with medication therapy monitoring, education, and collaboration with care teams.

The position operates in a hybrid model (approximately 25% onsite, 75% remote) after training, with a focus on delivering high-quality pharmaceutical care and chronic disease management within CVS Health’s patient-centered programs.

Qualifications

• Current Pharmacist License in good standing in Arizona.
• Reside in Arizona within commutable distance of Chandler.
• Six months pharmacy experience.
• Six months experience with Windows-based applications (Windows, MS Office, internet, email).

Responsibilities

• Reviews and interprets specialty medication orders, ensuring accuracy and adherence to guidelines.
• Establishes specialized patient counseling and education on administration, side effects, and therapeutic goals.
• Confers with health-care providers and insurers to obtain prior authorizations and facilitate reimbursement.
• Conducts complex medication assessments, identifying interactions and contraindications for specialty meds.
• Assesses patient response and adjusts treatment plans accordingly.
• Educates providers on specialty medications, dosing, interactions, and alternative therapies.
• Integrates with multidisciplinary care teams and contributes to treatment decisions.
• Ensures compliance with regulatory requirements, quality standards and safety protocols.
