We are hiring a Social Worker who is passionate about supporting youth and families through a highly individualized model of mental health care that consists of strength-based and family-centered support. As a Social Worker, you will meet with youth and caregivers on a weekly basis to provide psychoeducation and coordinate between providers. Services for a caseload of 8-10 clients will take place in-person within the community (Petaluma/Santa Rosa), while other work can be completed remotely.

About Kaiser Wraparound

This Kaiser Wraparound program is a partnership between Kaiser Permanente and Seneca Family of Agencies, providing behaviorally focused mental health services to children and families. The program aims to provide children and their families with the support and services they need to maintain the lowest level of care possible and avoid out-of-home placements (group homes, juvenile hall, hospitalization). Team members work with children and families providing services almost exclusively in the community: in families’ homes, schools, and other community locations.

Responsibilities
  • Identify engagement needs for each youth and family and work with Wraparound team members to build connections.
  • Provide assistance in case planning and consultation on clinical and casework issues.
  • Complete mental health notes, assessments, treatment plans, outcome measures, incident reports, and other documentation accurately and promptly.
  • Provide individual or family therapy when indicated.
  • Participate in 24-hour on-call support system.
  • Function as team clinical leader for your caseload, upholding wraparound principles.
  • Maintain availability for crisis intervention services.
  • Participate in supervision sessions.
Qualifications

Required

  • Master’s degree in social work, psychology, or counseling.
  • Registered with California BBS: ASW, AMFT, APCC, or fully licensed.
  • Flexible schedule, including some evenings and weekends.
  • Part of a rotating emergency on-call system.
  • At least 21 years of age.
  • Valid driver’s license, clean driving record, insurability.
  • TB test clearance, fingerprinting, and other licensing/certification requirements.

Preferred

  • Bilingual Spanish skills.
  • Experience working with children in out-of-home care.
  • Excellent organizational and critical-thinking skills.
Schedule
  • Full-time options: Monday-Friday 9am-5pm, or Monday-Thursday/Tues-Fri 9am-7pm.
  • Hybrid model: in-person and remote work, schedule determined with supervisor.
Benefits
  • Salary: $75,000 - $89,000/year, based on experience, with additional increases for licensure and bilingual proficiency.
