Job summary

Ponos Care is seeking an RN Case Manager to join a hybrid team delivering telehealth, home visits, and LTSS coordination for Medicaid populations. The role emphasizes comprehensive assessments, care planning, and collaboration with interdisciplinary teams to promote independence and health equity.

Responsibilities include education, care coordination across states, and ensuring compliance with regulatory standards.

Qualifications

• Bachelor’s degree in nursing (BSN) or equivalent required.
• Active Illinois RN license of practice.
• CPR/BLS certification required.
• Minimum 2+ years of clinical nursing experience in care management, population health, acute care, hematology, pain management, or community health.
• Experience serving Medicaid or complex care populations is preferred.

Responsibilities

• Conduct comprehensive member assessments via telephonic outreach or in-home visits.
• Develop and implement individualized care plans with the interdisciplinary team.
• Provide preventive and rehabilitative nursing and education to promote self-management.
• Coordinate clinical services and referrals across care continuum.
• Ensure documentation meets CMS, NCQA, HEDIS requirements and HIPAA standards.
