Job summary

Community Health Network is seeking an Ambulatory Triage Registered Nurse to join the Family Medicine Care team in Anderson, IN. The RN will perform phone triage, patient education, scheduling, and care coordination, with a hybrid work arrangement after training including onsite and remote work from home.

The role requires Indiana RN licensure, 3+ years of nursing experience preferred, and EPIC experience is a plus.

Qualifications

• RN license in Indiana is required.
• 3+ years of nursing experience preferred.
• Licensed as RN with Indiana license in NLC.
• Experience in a medical office/outpatient setting is preferred.
• Phone triage experience is a plus.

Responsibilities

• Perform phone triage and patient education.
