Job summary

Infinity Hospice Care LLC in Texas is seeking a Speech-Language Pathologist to provide coordinated care for clients of all ages, with a focus on home health and hospice services. The position is primarily remote covering our Texas service areas, with in-person visits when distance or patient needs require.

You will perform assessments, plan and revise care plans, document progress notes, and communicate with families and care teams.

Qualifications

• Master’s or doctorate in Speech-Language Pathology from an accredited program.
• Current state SLP license in good standing.
• Two years of clinical SLP experience; home health preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide coordinated care and implement patient care plans.
• Conduct clinical assessments and document Plans of Care.
• Deliver in-person visits when possible and remote visits as needed.
• Communicate effectively with patients, families, and health care team.
• Ensure compliance with third‑party payers and company policies.
• Maintain ongoing professional development and continuing education.
