PeopleOne Health, Inc. is seeking a Registered Dietitian to engage with members in-person, virtually, or by phone to provide medical nutrition therapy.

You will conduct nutrition assessments, develop diet plans, counsel on healthy eating, and coordinate care with the interdisciplinary team using EMR documentation.

This hybrid/remote role offers flexibility to meet client schedules and a supportive environment focused on preventive care and positive health outcomes.

Qualifications

• Bachelor’s degree or higher in nutrition or equivalent; Master’s preferred.
• Current CDR registration and state licensure required.
• 1-2 years clinical experience desirable; knowledge of chronic disease management a plus.

Responsabilités

• Conduct nutrition assessments to identify nutritional needs and plan care.
• Provide nutrition therapy based on evidence-based standards.
• Counsel individuals and groups on healthy nutrition and disease management.
• Coordinate care with physicians and interdisciplinary team.
• Document assessments, interventions and plans in EMR.
• Act as nutrition liaison for committee work and company initiatives.
• Prepare materials and presentations with accessible health literacy.
• Contribute to quality improvement and maintain a supportive environment.
• May work outside regular hours to accommodate client schedules.
