Job summary

Geode Health in Illinois is seeking an experienced Psychiatric PA or PMHNP to join our hybrid Mental Health Provider team. You will conduct initial evaluations, develop treatment plans, and provide medication management with psychotherapy as needed.

You will collaborate with psychiatrists and therapists in an integrated care model, while delivering care through onsite and telehealth visits. A data-driven, measurement-based approach informs every patient plan.

Qualifications

• Active psychiatric PA or PMHNP license in IL.
• Active DEA license.
• Active CDS license.
• Excellent clinical knowledge and communication skills.
• Proficiency with EMR and up-to-date electronic health records.
• Ability to evaluate a broad spectrum of mental health conditions.
• Geode prefers APPs with 1+ year outpatient experience.

Responsibilities

• Provide psychiatric evaluations and develop individualized treatment plans.
• Medication management and supportive psychotherapy as clinically appropriate.
• Collaborate with psychiatrists and mental health therapists to coordinate care.
• Deliver care in a hybrid model with onsite and telehealth sessions.
• Utilize measurement-based care to track patient outcomes and adjust plans.
• Maintain accurate documentation in the EMR for all encounters.
