Marin Community Clinics seeks a Hybrid Primary Care Practice Partner to deliver high-quality, evidence-based primary care through a mix of in-person and virtual visits. You will collaborate with designated PCPs, manage urgent care needs, and support transitions of care for patients transitioning from acute to home.

California-licensed Advanced Practice Providers with EPIC experience are encouraged to apply.

Formación

• Minimum 3 years of experience providing primary care in family or community health setting.
• Advanced Practice Provider (NP/PA) licensure in California (CA medical license) and DEA certificate required.
• Experience with EPIC electronic health record.
• Prior experience in a Federally Qualified Health Center (FQHC) is a plus.

Responsabilidades

• Provide high quality, evidence-based virtual primary care in collaboration with in-clinic PCPs.
• Conduct thorough virtual history and assessment; develop treatment and follow-up plans.
• Perform urgent care and medical triage visits (in-person or virtual).
• Support transitions of care by addressing urgent needs for patients transitioning from acute care to home.
• Offer patient education, counseling, and follow-up to promote wellness and chronic disease management.
• Manage EHR inbox and coordinate care with abnormal results; document in EHR timely.
