Job summary

Somatus is hiring a Patient Health Advocate (PHA) to support CKD/ESKD patients through care coordination, education, and caregiver/provider collaboration. The PHA will spend time in physician offices and conducting home visits to ensure seamless transitions of care and adherence to treatment plans.

You'll work in a Hybrid Telehealth environment, partnering with physicians, nurses, and office staff to improve patient outcomes and reduce readmissions.

Qualifications

• High school diploma or equivalent required.
• 1+ years of experience in case management or care management.
• 2+ years healthcare related experience.
• Experience with Medicare, Medicaid, and/or Special Needs populations.
• Experience working in a physician office.

Responsibilities

• Engage patients and physician practices to coordinate care.
• Work from physician offices and home visits as needed.
• Support regular interdisciplinary care team meetings.
• Serve as primary contact for provider practice regarding patient needs.
• Follow up on health management plans in coordination with RN/Care team.
• Educate providers about the Somatus program and workflows.
• Assist patients with transitions of care and scheduling follow-ups.
