Claxton Dietetic Solutions, LLC is seeking a Registered Dietitian to provide clinical nutrition services in long-term care and rehabilitation settings. This part-time role is approximately 27 hours per week, with potential to split into two openings: 11 hours and 16 hours.

A hybrid work arrangement may be considered based on facility needs, with onsite visits during weekdays and a focus on nutrition assessments, care plans, and nutrition risk monitoring.

Qualifications

• Registered Dietitian credential (CDR) required.
• Active state licensure or certification, as applicable.
• Strong organizational and time-management skills.
• Long-term care or geriatric experience preferred, but not required.

Responsabilités

• Conduct nutrition assessments and documentation.
• Complete MDS assessments and care plans.
• Monitor weights and nutrition risk.
• Participate in nutrition at-risk meetings.
• Perform meal rounds and sanitation audits.
• Onsite visits completed during weekdays.
