Job summary

PAX Health, LLC, a leading mental and behavioral health organization, is seeking a New York licensed LCSW to join an outpatient treatment team in a hybrid model with offices in Manhattan, Brooklyn, The Bronx, Long Island, and New Jersey.

The candidate will conduct assessments, develop treatment plans, provide individual sessions, and coordinate with a multidisciplinary team within the Workers' Compensation context.

Qualifications

• Licensed in New York State (LCSW)
• Minimum 5 years of experience as a fully licensed LCSW
• Ability to provide 30-minute sessions in Workers' Compensation context
• Registration/certification by state/national authority required

Responsibilities

• Receive and respond promptly to intake referrals for workers' compensation cases
• Conduct Bio/Psych/Social assessments and formulate treatment plans under supervision
• Meet with patients for individual sessions as progress dictates
• Complete aftercare and discharge planning
• Provide case management and referrals to outside resources when needed
• Maintain accurate client records in the Electronic Health Record (EHR)
• Attend treatment team meetings and supervision with Clinical Director
• Collaborate with multidisciplinary team to deliver full spectrum clinical services
• Adhere to confidentiality laws and agency ethics and policies
