Job summary

Meta Care Inc. is seeking a Nutrition Coach in a hybrid role combining remote coaching with on-site engagement, including cooking classes for members. The specialist will guide clients in healthy mindset development and nutrition choices, aiming for lasting health changes.

The role emphasizes coaching, data collection, and collaboration with multidisciplinary teams to ensure continuity of care and progress toward individual goals. Strong communication and tech-savvy capabilities are required.

Qualifications

• Proven experience as a nutritionist.
• Strong proficiency in creating and implementing care plans.
• Excellent communication and organizational skills.
• Typing speed minimum of 80 wpm and tech-readiness.

Responsibilities

• Assesses, identifies, and addresses the nutritional needs of its members.
• Demonstrates a broad nutritional knowledge of disease states including diabetes, obesity, and other medical conditions.
• Leads onsite cooking classes and hands-on culinary demonstrations to help members apply nutrition guidance to real-world meal preparation.
• Develops and implements group programs for high-risk populations.
• Collects data relating to target population to evaluate the program and outcomes.
• Participates in QI initiatives.
• Must see a minimum of 8 members per day.
• Travel to on-site conferences, meetings, and cooking classes as required.
• Develop quarterly business reviews that summarize work for the past 90 days, current, and projected.
• Perform other duties as requested.
