Alignment Health is pioneering home-based senior care with a CAW program that integrates in-home assessments, telehealth, and a collaborative care team. This Advanced Practice Clinician role emphasizes proactive, compassionate care for vulnerable members with a balanced hybrid schedule.

You will work two days in the field and three days remotely, building care plans with an interdisciplinary team to reduce gaps in care and improve outcomes. Strong credentials and licensing are essential.

Qualifikationen

• Master's degree from an accredited NP or PA program.
• Active California NP or PA license.
• Active NP or PA board certification.
• Current NPI number, DEA registration, and BLS certification.
• Valid driver's license and automobile insurance.

What You'll Do

• Conduct in-home assessments, medication reviews, and health screenings for Alignment members two days per week.
• Provide telehealth support remotely from home three days per week.
• Educate patients and caregivers by aligning care plans with individual health goals and needs.
• Identify diagnoses appropriate for care management and active medical management.
• Partner with the Regional Medical Officer and interdisciplinary team to develop individualized care plans.
• Coordinate care across multiple providers, including primary care physicians, specialists, and ancillary providers.
• Lead and collaborate with a broader clinical team that may include nurses, health coaches, social workers and care coordinators.

Minimum Qualifications

Education

• Master's degree from an accredited Nurse Practitioner Program or Physician Assistant Program.

Licensure & Certifications

• Active California Nurse Practitioner License or California Physician Assistant License.
• Active Nurse Practitioner Board Certification or Physician Assistant Certification.
• Current NPI Number.
• DEA Registration.
• Current BLS Certification.
• Valid Driver's License and current automobile insurance.

Preferred Qualifications

• One (1) year of prior clinical or home care experience.
• Experience working with an electronic medical record (EMR) system.
• Experience caring for older adult and geriatric populations.

Pay Range: $130,332.00 - $195,498.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
