Licensure & Certifications* Active California Nurse Practitioner License or California Physician Assistant License.* Active Nurse Practitioner Board Certification or Physician Assistant Certification.* Current NPI Number.* DEA Registration.* Current BLS Certification.* Valid Driver's License and current automobile insurance.
