Qualifications

• 1+ year of experience in Geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsibilities

• Deliver comprehensive preventative care to residents in skilled nursing facilities.
• Conduct on-site visits and provide patient-centered care, supported by remote monitoring and chronic care management.
• Build strong relationships with facility staff and patients, maintaining a patient-centered approach.
• Collaborate with clinical directors, regional managers, and supervisors to ensure high-quality care.
• Participate in quality improvement initiatives to minimize hospitalizations and elevate patient care standards.
