Job summary

Arcadia on the Hill in Springfield, IL is seeking an MDS Coordinator for an onsite/hybrid role to oversee MDS documentation, care planning, and reimbursement processes. You will coordinate with nursing, dietary, social, restorative, rehabilitation and physician teams to ensure accurate assessments and timely submissions.

The position requires LTAC/LTC experience, active RN or LPN licensure, and restorative certification within six months.

Qualifications

• RN or LPN with current unencumbered state licensure.
• Long Term Care experience required.
• Restorative Certification within six months of hire.
• Ability to read, write, speak and understand English.

Responsibilities

• Assesses health status and level of care for new admissions.
• Ensures timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Maintains schedule of MDS assessments with proper reference dates.
• Completes care plans on admission, quarterly and as needed per regulatory timeframes.
• Schedules and conducts resident care conferences and coordinates interdisciplinary reviews.
• Oversees restorative nursing measures on care plans.
