Urban Health Plan (UHP) seeks a Community Health Worker for the Perinatal Infant Community Health Collaborative (PICHC) to support maternal and infant health by addressing disparities and linking birthing people to essential care and resources. The role involves referrals, care coordination, and advocacy, with a mix of onsite and remote duties including field visits.

The position supports pregnancy, postpartum transitions, breastfeeding, and infant care education, with anticipated collaboration

Formación

• Associates Degree required.
• Passion and dedication to maternal and infant health.
• Ability to conduct outreach and home visits.
• Excellent computer-based skills including Outlook, Word, PowerPoint and ability to maintain participant data in software.
• Bilingual in English and Spanish is required.
• Team player.

Responsabilidades

• Provide basic health education about pregnancy, postpartum, breastfeeding and maternal care.
• Conduct community outreach, tabling events, regular home visits and service coordination.
• Lead health workshops in office and in home-based settings (prenatal care, immunization, breastfeeding, family planning).
• Provide referrals to supports, services, entitlements and benefits (including WIC, health insurance).
• Enter client data into software daily.
• Assist with special projects as needed.
