Job summary

Vynca Inc. in Sacramento, CA is seeking a Clinical Lead Care Manager to serve as the client’s main contact and coordinate with doctors, specialists, and social services to align on care needs. This hybrid role requires travel within Sacramento County up to 5 days a week.

A California LVN license is required, with 2+ years in care management and bilingual Spanish/English. Proficiency with EMR/CRM and Google Workspace is expected to support care coordination and documentation.

Qualifications

• Active California LVN license required.
• Willing to work Monday-Friday 8:30am-5:00pm in field and remote.
• 2+ years in care management, care navigator, or community health work.
• Bilingual Spanish/English required.
• Knowledge of resources related to social determinants of health.
• Excellent oral and written communication skills.
• Positive interpersonal skills.
• Clean driving record and reliable transportation.
• Proficient with Google Workspace, MS Office, and internet.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals.
• Oversee development of client care plans and SMART goals.
• Provide services where the member resides, including office, telehealth, or field-based.
• Connect clients to social services and supports as needed.
• Advocate on behalf of the client with health care professionals.
• Utilize evidence-based practices and document progress in CRM/EMR.
• Complete monthly reporting to ensure program compliance.
• Attend required trainings.
