Job summary

Qualifications

• Active California LVN license required.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services.
• Develop client care plans and set SMART goals.
• Offer services where the member resides, seeks care, or finds most accessible (office, telehealth, or field).
• Connect clients to necessary social services and supports.
• Advocate on behalf of the client with health care professionals (e.g., PCP).
• Utilize evidence-based practices, including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
