Job summary

Vynca in Sacramento is seeking an experienced Clinical Lead Care Manager (CLCM) to join our team. Under the direction of the ECM Clinical Manager, the CLCM serves as the client’s primary contact and coordinates with doctors, specialists, pharmacists, social services, and others to align on care needs.

This hybrid role travels throughout Sacramento County up to 5 days per week, with residents within 25 miles favored.

Qualifications

• California license required (ACSW/LPCC/LCSW/LMFT).
• 1+ year of experience as a care manager; 2+ years preferred.
• Available Monday–Friday 8:30am–5:00pm, field and remote with some evenings/weekends.
• Valid driver’s license and reliable transportation.
• Bilingual preferred (English/Spanish).

Responsibilities

• Assess member needs across physical, mental health, social supports, housing, and access to community services.
• Develop and monitor client care plans and SMART goals.
• Provide services in member's residence, telehealth, or office-based settings.
• Coordinate with family caregivers and care team for wellness and independence.
• Advocate with healthcare professionals to ensure appropriate services.
• Document services and outcomes; maintain EMR records and CRM entries.
• Complete monthly program reporting and attend trainings.
