Insightful Therapy Solutions is seeking a Licensed Clinical Social Worker (LCSW) to join our hospice team in Modesto, California. This hybrid role combines secure video sessions and in-person visits at our Hospice Center, delivering compassionate mental health support to individuals facing end-of-life challenges.

The ideal candidate holds an MSW and a California LCSW license, with strong assessment, treatment planning, and collaboration skills.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current Licensed Clinical Social Worker (LCSW) credential in California with no disciplinary actions.
• Extensive clinical experience in social work, including assessment, diagnosis, and therapeutic intervention processes.
• Exceptional interpersonal and communication skills, with a demonstrated ability to build rapport with a diverse range of clients.
• Technologically savvy and comfortable utilizing virtual platforms for client interaction.

Responsibilities

• Individual Counseling: Conduct in-depth counseling sessions via video chat and face-to-face appointments.
• Assessment and Treatment Planning: Thoroughly assess client needs and craft customized treatment plans.
• Client Advocacy: Serve as an advocate for clients and help set achievable goals.
• Collaboration: Work with nurses, physicians, and spiritual care staff to develop care plans.
• Resource Referrals: Guide clients and families to navigate community resources.
