Carease Health in Cheswick, PA is seeking an experienced Adult Nurse Practitioner to join our nationwide preventive care team serving residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

You will collaborate with facility staff and our clinical leadership, deliver high-quality preventive care, maintain documentation, and participate in quality improvement initiatives.

Formación

• Active Adult NP license in PA
• 1+ year of experience in geriatric care or internal medicine
• Excellent communication, collaboration, and teamwork skills
• Ability to maintain confidentiality and handle sensitive information

Responsabilidades

• Provide high-quality preventative care to residents in skilled nursing facilities
• Conduct on-site visits and remote work from home
• Utilize RPM, CCM, and CoCM to enhance patient care
• Collaborate with facility staff and Carease Health clinical team
• Maintain accurate and timely documentation
• Participate in quality improvement initiatives to elevate patient care standards
