You will coordinate services, advocate for clients, and partner with jail, courts, and community providers to divert from incarceration through personalized service plans.

Responsibilities include direct client support, face-to-face meetings, documentation in electronic health records, and ensuring HIPAA

Qualifications

• Bachelor’s degree required; master’s degree preferred.
• Two years of case management or behavioral health experience.
• Knowledge of mental health and addiction treatment principles.
• Strong organizational and communication skills.

Responsibilities

• Serve as primary advocate for individuals and families, aligning services with needs and goals.
• Develop and update individualized service plans based on assessments.
• Provide face-to-face client support at least monthly.
• Assist clients in navigating the criminal justice system, including court proceedings.
• Coordinate with behavioral health providers to ensure access to services.
• Facilitate interdisciplinary team meetings and advocate for resources.
• Collaborate on discharge plans and post-release services.
• Liaise between clients, providers, and systems for care coordination.
• Maintain accurate electronic health records and ensure HIPAA compliance.
• Participate in audits and quality reviews.
