Job summary

Continuum of Care in Middletown, CT is hiring a Direct Support Professional for a 40-hour FT role, Tue-Sat 3 pm-11 pm. The position offers a base rate of $23.28/hour with a $1 weekend differential to $24.28/hour.

The DSP will support individuals with Intellectual Disabilities, helping them live in their own homes, engage in daily activities, and participate in community life, under IP and behavioral plans.

Qualifications

• Bachelor’s or Associate’s preferred; High School Diploma or equivalent considered with related experience.
• Valid Connecticut Driver’s license with clean driving record required.
• Certification as a Direct Support Professional preferred.
• CPR & First Aid, PMT, and DDS/CoC certifications required or completed.
• Computer skills to navigate agency electronic records, email, and documentation.
• Evenings, weekends, and overnights availability.

Responsibilities

• Provide client care and activities of daily living.
• Establish rapport and maintain professional boundaries.
• Face-to-face hands-on support enabling individuals to live in their home.
• Deliver services per IP and behavioral programming.
• Encourage natural supports and community involvement.
• Input data in Therap or designated systems and track daily data.
• Attend staff and designated meetings; complete other client care tasks.
