• Data analysis skills to turn insights into actionable solutions.
• Ability to prioritize multiple competing priorities and adapt to diverse populations.
• Project management training completed or willingness to complete within six months.

Responsibilities

• Collaborate with leadership to champion standard improvement activities across post-acute providers.
• Lead transformation projects tied to payor initiatives and the triple aim.
• Manage projects using formal PM tools to coordinate planning and tracking.
• Analyze multiple data sources and craft summaries to support providers in improving performance.
• Deliver training and education on program requirements, resources, data collection, and quality standards.
• Prepare proposals, reports, and summaries for assigned projects and meet deadlines.
• Engage in sharing opportunities related to improvement initiatives and learnings.
