Mercy Clinic Primary Care - Eureka is seeking a Primary Care Nurse to work with Mercy Primary Care Providers to deliver safe, timely, patient-centered care and drive quality outcomes for an attributed panel.

The role begins with on-site training for three months, followed by a 50/50 hybrid schedule. Full-time coworkers work on-site five days per pay period and monthly on-site clinic meetings are required. Proficiency with Epic EMR and triage skills are highly preferred.

Qualifications

• Diploma in Practical Nursing required.
• Active LPN license required.
• RN path includes ASN/BSN option.
• 2–3 years of relevant experience preferred.

Responsibilities

• Provide patient-centered care under Mercy Primary Care Providers.
• Collaborate with patients, providers and the care team to drive quality outcomes.
• Attend monthly on-site clinic meetings and support clinical operations.
• Perform computer-based tasks and utilize Epic EMR efficiently.
