Overview

THIS ROLE IS REMOTE, STAFF MEETINGS ARE CONDUCTED ONSITE IN CHICAGO, IL.

• Monday through Thursday from 12:00 pm - 8:00 pm
• Friday from 9:30 am - 6:00 pm
• Pay Rate between $20.00-$29.00 (Commensurate experience)

Summary

The Community Health Worker provides support to the IL Medicaid Care Coordination team by outreaching to IL Medicaid members to complete their initial health risk screening and completing door knocks when necessary to contact hard to reach members.

Requirements

• Bachelor\'s degree in Healthcare Administration, Social Work or related field
• Community Health Worker (CHW) certification from HCSC approved college
• 4 years’ health care, social services or community advocacy experience
• 1 year of experience coordinating member related needs, providing assistance to members, and analyzing member needs
• Knowledge of medical terminology
• PC proficiency including Microsoft Office applications
