Qualifications

• 5+ years of experience in a clinical, non-profit, or managed-care organization.
• Extensive knowledge of health equity and social drivers of health.
• Experience using translation services preferred.

Responsibilities

• Manage incoming referrals for enhanced HRSN care management.
• Provide longitudinal care management for Members.
• Conduct outreach and HRSN screenings.
