Position Overview:

We are seeking a Hospice RN Case Manager to provide high-quality, patient-centered care to individuals in the Kalamazoo and Battle Creek areas. This is a full-time, weekday-only position that is a combination of field (visiting patients in their homes) and remote, offering flexibility and autonomy in your schedule.

Key Responsibilities:

• Manage a caseload of approximately 10 patients, averaging 3–4 visits per day
• Travel to patient homes to assess and deliver hospice care
• Perform wound care, medication assessments, and symptom management
• Collaborate with interdisciplinary team members to ensure comprehensive care plans
• Provide emotional and clinical support to patients and their families
• Visit the local office only as needed (e.g., to pick up supplies)
• Mileage Reimbursement: $0.67 per mile
• Cell Phone Stipend Provided
• Flexible start-from-home model
• Supportive team environment with no on-call or weekend duties

Qualifications:
