Are you looking for an opportunity to advance your career while working with an extraordinary team? At Merakey, we put heart and soul into everything we do. We are seeking a Transition Blended Case Manager to join our team in our Cambria County, PA location.

Salary: Union $18.40 with a 6 month and 1 year increase

Position Details:

• Under the direction of the Blended Case Management (BCM) Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan.
• This position will be based home but will have 12-15 youth and young adults (15-26 years of age) per week.
• Case load will be located in Cambria County (mileage will be paid).
• As a member of the BCM team, this position will serve as the primary contact for youth and young adults.
• Core responsibilities will include providing case management, service planning, and coordination of services with other agency providers to meet the clients’ needs along with supportive counseling.
• Will include on-call responsibilities; rotation approx. 1 week a month with a $75 stipend.

Qualifications

Our ideal candidate will possess the following skills:

• Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or, Registered nurse.
• Possess a current PA driver’s license with 2+ years of driving experience with no suspensions.
• A Reliable vehicle (this is a driving position).
• Current car insurance (required).
