Job Description

Job Title: Hybrid Behavioral Health Professional

Location: Remote with up to 50-75% travel to long-term care facilities, Bureau County, IL

Pay: $38.00 - $40.00 per hour

Scheduled Pay: Bi-Weekly

Hours: Monday – Friday, 8:00 am - 5:00 pm

Job Type: Full Time

Responsibilities:
  1. Coordinate case management activities for Medicaid Long Term Care/Comprehensive/DSNP Program enrollees.
  2. Utilize critical thinking and judgment to collaborate and inform the case management process, facilitating appropriate healthcare outcomes by providing care coordination, support, and education to members using care management tools and resources.
  3. Conduct comprehensive evaluations of members using care management tools and data review.
  4. Coordinate and implement assigned care plan activities and monitor progress.
  5. Conduct multidisciplinary reviews to achieve optimal outcomes.
  6. Identify and escalate quality of care issues through established channels.
  7. Use negotiation skills to secure appropriate options and services to meet members' benefits and healthcare needs.
  8. Apply influencing and motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes.
  9. Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices.
  10. Assist members in actively participating with their providers in healthcare decision-making.
  11. Monitor, evaluate, and document care in compliance with regulatory guidelines and company policies.
Requirements:
  • Master's degree in psychology or social work.
  • Minimum of 1 year experience in behavioral health, social services, or a related field.
  • LCSW or LCPC certification required.
  • Case management experience is required.
