Job summary

DePaul Community Resources is seeking a full-time ADS Case Manager to support Roanoke and Christiansburg regions, 37.5 hours per week. The role focuses on documenting case records, coordinating with referral sources, and assisting families in the Agency Directed Services program.

The position is based mainly in Roanoke with a hybrid option, and involves collaboration with nurses, social services, and care attendants, while not performing field duties.

Qualifications

• Associate’s degree required and experience in human services preferred.
• Knowledge of Medicaid waivers and agency directed services is beneficial.
• Strong communication skills and professional composure required.

Responsibilities

• Create and maintain caregiver profiles in CareSmartz 360.
• Prepare intake paperwork and other visits.
• Upload approvals/denials and align with finance for authorizations.
• Interact with referral sources about services and Medicaid eligibility.
• Support staff and families with documentation and scheduling.
• Attend meetings and assist with program growth and resources.
• Assist with staff interviews and drug screens as needed.
• Meet regularly with program director to discuss referrals and service delivery.
