Job summary

Sage Eldercare Solutions seeks a Hummingbird Project Manager to lead our Hummingbird Project team, guiding therapeutic activities and QoL planning for older adults. You will supervise an Activity Specialist team and deliver direct in-home client services with strong community engagement.

Ideal candidates have licensure as Mental Health Professionals or equivalent Therapeutic Arts background, a Bachelor’s degree, 5+ years with older adults (3+ in supervision), and superior leadership and

Qualifications

• A licensed Mental Health Professional or comparable training in Therapeutic Arts.
• Bachelor's degree is required; Master's degree preferred.
• At least five years with older adults, including at least three years in supervision.
• Strong leadership, organization, and communication skills; creative problem solving.

Responsibilities

• Lead and supervise a team of Activity Specialists.
• Develop and update Quality of Life Plans for clients.
• Provide direct, billable services including in-home assessments and activity planning.
• Build relationships with community members and professionals to improve service.
• Ensure training and professional standards are met for the team.
• Engage in professional forums and community outreach to promote the project.
