Job Description - Human Services Coordinator I (260004NG)

Overview

The Department of Developmental Services (DDS) offers a career growth opportunity to a motivated and talented individual for the Human Services Coordinator I (HSC I) position in the South Valley Area Office in Southbridge.

The Human Service Coordinator I is the primary contact for individuals and families served by the Department and is expected to effectively represent the Area Office by building respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Area Office and Department. This position requires travel as needed to the greater Milford, MA area.

The HSC I coordinates and facilitates the development of Individual Service Plan (ISP) for individuals on an assigned caseload, monitors implementation of services provided by DDS funded residential, day and support program providers, and ensures individuals receive appropriate services through arranging, organizing and coordinating funded and generic support services in response to individual needs, while maintaining communication and outreach with community services and other state agencies.

Responsibilities
  • Monitor the safety and well‑being of assigned individuals
  • Provide advocacy in human, civil and legal rights
  • Facilitate team meetings
  • Monitor follow‑through on team recommendations while maintaining individual’s records and relevant computer databases
  • Maintain regular and consistent contact with individuals, service providers and families
  • Arrange or provide crisis intervention for individuals as needed, assess needs and make recommendations for follow‑up planning
  • Provide information to supervisor and other members of the Area Office Management Team as needed
  • Perform other related duties which may include individuals engaged in Participant Directed Services
Schedule

Full‑time, 37.5 hour position. Days off: Sunday, Saturday. Flexibility required to meet the needs of the Area Office.

Qualifications
  • At least three (3) years of full‑time or equivalent part‑time, professional experience in human services work or social work.
  • Alternatively, any equivalent combination of the required experience and substitutions below.
  • Substitutions: A Bachelor's degree or higher in a related field may substitute for two (2) years of the required experience.
  • Travel may be required. Incumbents who use a motor vehicle must have a valid driver’s license at the class level specified for assignment.
  • For Qualified Intellectual Disabled Professionals, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or a related field is required.

Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure. For all bargaining unit positions (non‑management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
