Job summary

U.S. Department of Veterans Affairs in Birmingham, AL, seeks a Social Worker for HUD-VASH within Social Work Service. The role provides outpatient social work services to Veterans and families, coordinating care and advocating for access to VA care and community resources.

The incumbent conducts assessments, plans interventions, and ensures proper documentation and eligibility, while participating in HUD-VASH program training and related duties at the Birmingham VA Health Care System.

Qualifications

• Knowledge of community resources and how to link Veterans to services.
• Ability to conduct psychosocial assessments and psychosocial interventions.
• Knowledge of medical/mental health diagnoses and treatment planning.
• Skill in coordinating services and referrals across agencies.
• Ability to provide consultation to new social workers and staff.

Responsibilities

• Provide outpatient social work in Birmingham VA Primary Care HUD-VASH program.
• Coordinate care and advocate for Veterans to ensure comprehensive service delivery.
• Perform HUD-VASH apartment site visits to ensure safe housing conditions.
• Verify Veteran status and eligibility for VA medical care and program participation.
• Document and report data in HOMES and participate in program training.
