The role focuses on helping residents access community services, linking them to case management, personal assistance, meals-on-wheels, transportation, counseling, preventive health screening, and legal advocacy to enable independent living.

Qualifications

• Bachelor's degree in Social Work, Gerontology, Psychology, Counseling or related field of study normally required.
• Minimum of three years' related experience normally required.
• ADA: Ability to bend, stoop, sit, reach, and lift items weighing up to 25 lbs.; visual and auditory skills.
