The HRSN Support Coordinator provides administrative coordination and direct client-facing support across the HRSN Tenancy Support and Outreach and Engagement (O&E) program in Marion and Polk counties. This position is funded through the Coordinated Care Organization (CCO) and operates within MWVCAA's Community Resource Program. The coordinator supports active caseloads, coordinates referral workflows, assists with member and landlord communication, and maintains data quality across program platforms. This is a grant-funded position through November 2027, with potential extension contingent upon continued funding.

Tuesday - Saturday. Day shift. Flexibility required.

MINIMUM QUALIFICATIONS

The requirements listed below are representative of the knowledge, skill, and/or ability required.

EDUCATION and/or EXPERIENCE
  • High School Diploma or GED
  • Associate's degree in Social Services or a related field preferred.
  • Three years of case management or social services experience required.
  • Experience with housing, sheltering, or homelessness-related programs strongly preferred.
  • Equivalent combination of education and experience will be considered.
CERTIFICATES, LICENSES, REGISTRATIONS
  • Driver qualifications are required for this position. Valid driver license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.
  • Candidate must pass a comprehensive MWVCAA background screening prior to employment.
  • Candidate must pass pre-employment and random drug screenings.
  • CPR/AED and First Aid Certification required within first 30 days of hire.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Knowledge of Housing First principles, trauma-informed care, and person-centered service delivery; understanding of social determinants of health and barriers to accessing services.
  • Familiarity with OHP/Medicaid systems and HRSN program requirements; knowledge of tenant rights, Fair Housing practices, and community-based resources in Marion and Polk counties preferred.
  • Proficiency in NeedNavigator, Unite Us, and Microsoft Office (Word, Excel, Outlook, Teams); ability to learn and adapt to new data and documentation systems.
  • Strong organizational and time management skills; ability to manage multiple tasks, meet competing deadlines, and maintain accurate records with consistent follow-through.
  • Effective oral and written communication skills; ability to work respectfully with diverse clients, landlords, partners, and internal staff.

WHAT YOU WILL DO TO SUPPORT THE COMMUNITY

This job description is not intended to be all-inclusive. Employee will also perform other reasonably related business duties as assigned by immediate supervisor and other management as required.

  • Accepts and assigns incoming HRSN referrals in Unite Us; monitors the referral queue and ensures timely distribution to HRSN Support Specialists in accordance with program workflows.
  • Assists HRSN Support Specialists with confirming OHP member eligibility for the HRSN Medicaid benefit and gathering required documentation for service authorization.
  • Supports members and HRSN Support Specialists with Tenancy Support activities, including collecting landlord documentation, communicating HRSN program information to landlords and property managers, and coordinating warm handoffs to community resources.
  • Conducts Outreach and Engagement (O&E) activities as assigned, including community outreach, needs screening, and connection of potential members to the HRSN program.
  • Enters and maintains accurate data in NeedNavigator, Unite Us, and other required platforms; supports data quality review and assists with quality control for team data entry.
  • Maintains organized and up-to-date client records and program documentation; supports program reporting activities and data disaggregation to identify service trends and gaps.
  • Communicates directly with members, landlords, and property management companies to follow up on service coordination, documentation, and outstanding needs.
  • Provides information about community resources and supports warm handoffs to internal MWVCAA programs and external partners to address whole-person member needs.
  • Responds to phone and email inquiries from members, partners, and program contacts; escalates complex issues to the ARCHES Program Manager as appropriate.
  • Participates in regular program meetings, trainings, supervision, and team collaboration; supports ongoing program development and process improvements.
  • Adheres to all HRSN program guidelines, MWVCAA policies, and PacificSource CCO requirements; maintain appropriate professionally boundaries at all times.
  • Participates in community outreach activities as assigned to support program awareness and member enrollment.
  • Maintains consistent attendance and punctuality; some evening or weekend hours may be required.
  • Participates in relevant agency meetings, trainings, and partner convenings as assigned.
  • Provides coverage for the Eviction Prevention lobby as needed by welcoming clients, responding to inquiries, conducting initial screenings, directing individuals to appropriate staff or resources, and ensuring a positive and professional customer service experience.

  • Ability to work effectively and maintain positive, professional relationships with team members and clients.

  • Specific vision abilities required include close vision and ability to adjust focus; manual dexterity for handling office equipment; occasionally lift up to 25 pounds.
  • Occasional driving in all weather conditions to client sites, partner locations, and agency offices in Marion and Polk counties.
WORK ENVIRONMENT

This position requires on-site work and is not eligible for remote work.

  • Primarily indoor, office-based work environment with regular community-based work at client homes, partner sites, and housing provider locations.
  • Some evening or weekend hours may be required; occasional remote work as approved by supervisor.
  • Close quarters, often with a client population experiencing homelessness, substance use disorders, and/or behavioral health challenges.
  • Occasional urgent situations may require coordination with law enforcement or emergency services.
