Job summary

Hyacinth Foundation in Paterson, NJ is seeking a dedicated professional to support Harm Reduction Services under the supervision of the Statewide Harm Reduction Manager. The role emphasizes community case management, outreach, and referrals to stabilize clients and connect them with housing, transportation, employment, and other supports.

Qualifications

  • Bachelor’s degree or relevant experience.
  • 1–3 years of experience in Harm Reduction/Social services/community services.
  • Excellent listening and written/verbal communication skills.
  • Familiarity with trauma-informed care approach.
  • Bilingual English/Spanish is a plus.

Job description

Under the supervision of the Statewide Harm Reduction Manager, performs duties related to the HRC specific community case management and referral services as needed.

Responsibilities
  • Client Services (70%)
    • Conduct community outreach, provider education, and client recruitment of individuals in need of Harm Reduction Services.
    • Offer case management services to address obstacles while stabilizing at the HRC.
    • Conduct assessments to determine clients’ needs of supportive services.
    • Refer clients to HRC staff and/or community agency staff to assist them with psychosocial supports (e.g., housing, transportation, employment).
    • Follow‑up with all referrals and clients to ensure all needs are being met.
  • Reporting/Monitoring (15%)
    • Establish, maintain and update client case records using approved agency forms and procedures. Ensure confidentiality of all records.
    • Prepare monthly reports in accordance with agency policies and procedures.
    • Maintain and update client information in required reporting system.
  • Hyacinth Representation/Advocacy (10%)
    • Attend and participate in staff meetings and supervisory conferences.
    • Maintain working relationships with other community providers.
    • Observe established administrative procedures.
    • Provide written reports and maintain statistics as required.
    • Represent the agency at community events, meetings, and conferences.
    • Attend and participate in staff training opportunities.
    • Adhere to Hyacinth policies and procedures including client confidentiality provisions.
  • Training and Certifications (5%)
    • Attend required opioid and harm reduction meetings.
Other Duties
  • Maintain working relationships with other community providers.
  • Observe established administrative procedures.
  • Assist in preparation of written and electronic reports, maintaining records and statistics of program data as required.
  • Assist with group activities, outreach events and other program informal and formal social events.
  • An ideal candidate must have flexibility to work nights and some weekends as needed.

  • Recognizes problems and complaints and escalates to team leaders and/or managers when necessary.
Requirements
  • Required Minimum Education: Bachelor’s degree, can be replaced with appropriate experience.
  • Required Years of Related Experience: 1-3 years, experience with Harm Reduction/Social services/community services.
  • Required Knowledge, Skills And Abilities:
    • Active listening skills.
    • Familiarity with trauma informed care approach.
    • Knowledge of available services and community resources.
    • Bi‑lingual (English and Spanish) is a plus.
  • People Management Experience: Not required.
  • Candidate must have excellent oral presentation and writing skills.
  • Expressed commitment to Hyacinth philosophy.
Preferred Qualifications
  • Preferred Minimum Education: Bachelor's degree.
  • Preferred Area Of Expertise: Experience working in Harm Reduction; knowledge of supportive services and community resources.
  • Preferred Related Industry Experience: Grant reporting and data collection experience.
  • Preferred Knowledge, Skills And Abilities: Must be knowledgeable about working with people with active substance use disorders; must be culturally competent and sensitive to the varying socio‑cultural factors that contribute to substance use; must be able to work non-traditional hours and days including late nights and weekends in order to reach high‑risk populations.
