Job summary

The Society of St. Vincent de Paul - Phoenix is seeking a Workforce Case Manager who will serve as a vital advocate and resource for program participants experiencing homelessness. You will guide individuals by developing service plans, connecting them with employment opportunities, and providing ongoing support.

The ideal candidate will have strong communication skills, be highly organized, and possess a passion for serving vulnerable populations. Experience in case management or human services is preferred. This position requires dedication and the ability to work independently while collaborating with team members.

Qualifications

• Minimum 1+ years experience in Case Management or 2+ years in a human services role.
• Ability to advocate effectively on behalf of residents.
• Experience working with vulnerable populations.

Responsibilities

• Carry a caseload of workforce participants, meeting with each resident at least weekly.
• Conduct assessments to determine clients’ needs and eligibility for programs.
• Monitor and document resident progress toward stabilization and housing goals.

Education

Bachelor’s degree in social work or a related field
