Responsibilities include housing searches, applications, landlord follow-ups, and tenancy support, while ensuring compliance with HIPAA and program guidelines.

Qualifications

• High School Diploma or GED required.
• Experience in IDD, behavioral health, housing services preferred.
• CPR/First Aid certification preferred or required per company policy.
• Ability to learn internal systems such as C2S.

Responsibilities

• Assist housing searches, applications, and transition processes with accuracy.
• Gather documentation, coordinate tours, and follow up with landlords.
• Support tenancy-sustaining activities and independent living skills.
• Document services in C2S and communicate with stakeholders.
