Job summary

Services for the UnderServed (S:US) is seeking a Diversion Specialist for our homeless prevention program in New York. The role includes direct intake of shelter applicants, developing housing plans, and coordinating services to promote housing stability.

Responsibilities include entering data into CARES, conducting home visits, and maintaining detailed case notes. A bachelor’s or related experience with strong knowledge of homelessness and related fields is required, with bilingual/cultural

Qualifications

• Bachelor's Degree in Human Service or related field, or High School Diploma/GED with 2 years relevant experience.
• Knowledge of homelessness, substance abuse, criminal justice, HIV/AIDS.
• Bilingual and/or bicultural preferred.
• Experience using Microsoft Office.

Responsibilities

• Assess shelter applicants referred from intake centers; serve as the first contact.
• Enter intake and referral information in CARES promptly.
• Develop individualized housing plans.
• Conduct home visits for furnished rooms/apartments.
• Provide general case management coordinating housing stability services.
• Meet with participants for follow-ups and recommendations.
• Maintain case notes in CARES.
• Participate in bi-weekly supervision.
• Other duties as assigned.
