Community Housing Network (CHN) is seeking a Housing Stability Coordinator to provide strength-based housing case management and empower participants within the coordinated entry system. The coordinator will work with program participants, partners, and team members to promote housing stability and positive outcomes.
The role involves assessing needs, developing individualized stability plans, linking participants to affordable housing opportunities, and documenting progress in required systems.

Reports to: Housing Stability Supervisor
TheHousing StabilityCoordinator is responsible for providing strength based, housing case management while engaging in compassionate and empowering problem-solving conversations with program participants, community partners, and team members to encourage housing stability. The coordinator will actively participate in the coordinated entry system as well as the larger service network as needed. The coordinator will also contribute to outcomes and continuous quality improvement at the agency and Continuum of Care level.
Travel required within the communities served.
CHN reserves the right to require a motor vehicle record check with respect to any employee where driving is an essential function of the position.
This position includes field-work, in-office work and remote work (from home office).
The primary function of this job requires travelling to community partner locations, shelters, community spaces, program participant homes, and meeting program participants in the field.
While performing the functions of this job, the position is both sedentary and at times requires the ability to be active including standing, walking in the outside elements, in and out of vehicle bending, lifting and/or arranging files and office products and supplies and may require employee to periodically stand on a short step stool to access files.
