Job summary

OCES is seeking a dedicated Housing Specialist to support older adults and individuals with disabilities in Brockton area locations, coordinating on-site services and linking residents with community resources.

You will lead multidisciplinary assessments, plan social activities, and communicate with residents and families; this role requires at least two years in human services, a degree in social work or related field preferred, and strong computer and communication skills.

Qualifications

• At least two years of experience in the human services field, preferably with the elder population.
• Bachelor's degree in social work or related field preferred.
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.
• Bilingual a plus; Haitian Creole or Cape Verde Creole preferred.

Responsibilities

• Review residents' needs to link community programs and services.
• Serve as a multidisciplinary assessment team leader and coordinate service coordination.
• Facilitate care conferences with residents, family, and participants.
• Plan and coordinate social, educational, and recreational programs for residents.
• Prepare and distribute a newsletter to inform residents of planned activities.

We are seeking a dedicated, caring Housing Specialist professional who enjoys working in the community with our Housing team to ensure services and support to residents on‑site in our Brockton area locations.

What You'll Do

• Review the needs of residents to link the community programs and services
