Delta-T Group is seeking a Housing Relocation Case Manager to help individuals and families experiencing homelessness secure and maintain stable housing in the Germantown/Mt. Airy area of Philadelphia.

The role requires a Bachelor's degree in Human Services and at least 1 year of case management experience, with strong written communication. Clearances (FBI, Child Abuse, PA State Background, Act 126), CPR, and First Aid Level 2 are required. Schedule is typically Mon–Fri, 9a–5p.

Qualifications

• Bachelor's degree in Human Services field (Psychology, Sociology, SW, Crim Justice, etc.).
• At least 1 year of case management experience.
• Strong written communication skills.
• Understanding of health system and local community partners.
• Pass FBI, Child Abuse, PA State Background Check, and Act 126 clearances.
• CPR certification and First Aid Level 2 certification.
• Ability to provide current medical documentation including Physical and TB test.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

The Life Skills Case Manager position will be responsible for the provision of case management services to individuals and families experiencing homelessness by providing support and resources to secure and maintain stable housing.

CLIENT'S REQUIRED SKILLS & EXPERIENCE

• Bachelor's Degree with major under Human Services umbrella (Psychology, Sociology, Social Work, Crim Justice, etc.)
• 1 year experience in case management
• Strong written communication skills.
• Strong understanding of the health system, community-based organizations, and partners in the designated service area(s).
• Ability to complete and pass clearances: FBI, Child Abuse, PA State Background Check, & Act 126.
• Provide documentation of: CPR Certification, First Aide Level 2 Certification.
• Must be able to produce current medical/health documents including: Physical examination, Tuberculosis (TB) test
