Start Corporation, a New Orleans nonprofit, seeks a Housing Specialist for its ACT team to support housing stability for individuals with behavioral health needs.

You will work on a multidisciplinary team, conduct assessments, coordinate housing referrals, and ensure documentation meets licensing and program standards in a community setting. You may drive client transport and assist in securing safe, affordable housing options.

Qualifications

• Bachelor's degree (preferred).
• Be over 18 years of age (required).
• Possess a valid driver's license (required).
• Minimum of one year experience with homeless population and housing experience (required).
• No restrictions or adverse actions that would disqualify for Medicaid or other billing (required).

Responsabilités

• Maintains professional and ethical standards of conduct at all times.
• Promotes recovery, personal choice, and respect.
• Interviews clients and/or families and completes assessments and other clinical documentation.
• Works as part of a transdisciplinary team and completes required documentation on all client services including data entry into required data systems.
• Ensures that clinical documentation and client files meet all applicable standards for licensing, accreditation, or other regulatory agencies.
• Performs regular case maintenance activities including documenting actions, making narrative entries, preparing necessary reports and/or social summaries, etc.
• Collects and analyzes data to identify needs for the assigned service program within the area.
• Provides referrals and linkages to community resources.
• Teaches social skills and household management skills.
• Maintains a clear driving record to drive company vehicles for client transportation.
• Attends ACT team meetings and supervision.
• Assists with obtaining and maintaining housing.
• Provides supportive services to increase skills for housing maintenance and community integration.
• Supports landlord recruitment and retention.
• Performs other duties as assigned by the Program Director.
