Job summary

Qualifications

• Completion of 30 semester hours or 45 quarter hours in Behavioral Sciences or related fields.
• At least 2,000 hours of supervised experience delivering services to adults or children with mental illness or SUD.
• 4,000 hours of experience delivering services to adults or children with mental illness or SUD is required.

Responsibilities

• Screen and complete intake in a timely manner per HUD and grant requirements.
• Develop and maintain HMIS documentation and client plans with quarterly reviews.
• Coordinate with housing authorities, treatment, and landlords to provide supportive housing services.
• Maintain client plans and ensure accurate records in EHR/HMIS within deadlines.

DescriptionAs a Partner in Wellbeing, the Homeless Services Specialist is responsible for assisting homeless clients with a mental illness, trauma, and/or chemical dependency to develop the skills and support necessary to acquire and maintain suitable, safe, and affordable housing. These services will be provided in the client’s home and community of choice primarily on a one-to-one basis but may also be provided in virtual settings.Conduct screening and intake process in a timely manner upon receiving a referral consistent with HUD and other grant requirementsComplete all intake and HMIS paperwork for the client and the respective funding sourceWork closely with local housing authorities, community action agencies, county social service agencies, treatment, correctional facilities, and landlords in the region to develop and provide many supportive housing services that meet the needs of the clients served by HIPMaintain documentation in the agency EHR and the HMIS system, correct any errors in HMIS entries, and have all documentation corrected and entered into HMIS within reporting deadlinesMonitor, review, and update each client’s plan at least quarterly with the assistance of the client and his/her community care teamRequirementsRelevant coursework by completing at least 30 semester hours or 45 quarter hours in Behavioral Sciences or related fields and having at least 2,000 hours of supervised experience in delivering services to adults or children with mental illness, substance use disorder, etc.Qualified through work experience 4,000 hours of experience in the delivery of services to adults or children with mental illness, substance use disorder, etc.Demonstrate knowledge of regional housing resources or the ability to learn themDemonstrate ability to work with individuals involved in the legal system who may have an active addiction, mental illness, or trauma without judgment and ability to meet the client where they are at in providing person-centered careMust pass a criminal background checkFree from substance use problems for a period of a least two years
