Job summary

The Ability Center of Greater Toledo is seeking a Community Supports Specialist to help people with disabilities navigate housing, healthcare, Medicaid waivers, and community resources. You will work with a Navigator team to create housing stability and independence, serving a 13-county area.

Responsibilities include conducting assessments, developing ILPs, coordinating services, and representing the center in community coalitions; bachelor’s degree required, MSW preferred, with a focus on

Qualifications

• Bachelor's degree in social work required; MSW strongly preferred.
• Five years of progressive experience in disability housing, healthcare, or related fields.
• Knowledge of Medicaid, waivers, and community resources is highly preferred.

Responsibilities

• Maintain a caseload of approximately 20–30 consumers.
• Conduct assessments and develop person-centered ILPs addressing housing, healthcare, and transportation.
• Assist clients in accessing affordable housing and housing resources.
• Provide follow-up support to promote housing stability and navigate benefits and services.
• Collaborate with Navigators and staff on complex cases.
