Job summary

The Ability Center in Ohio is seeking a Community Supports Specialist to help people with disabilities navigate housing, health care, benefits, and community resources. You will work with our Navigator team to assess needs and develop person-centered ILPs, connecting individuals to safe, affordable housing and supports.

This role emphasizes independence, SDOH, Medicaid, and cross-system coordination across a 13-county area.

Qualifications

• Bachelor's degree in social work required; MSW strongly preferred.
• Five years of experience in disability or housing services.
• Experience with Medicaid, waiver services, and public benefits preferred.
• Knowledge of housing systems and Social Determinants of Health (SDOH).
• Ability to coordinate services across multiple organizations using a person-centered approach.

Responsibilities

• Maintain a caseload of 20-30 consumers receiving ongoing support.
• Conduct comprehensive assessments and develop ILPs addressing housing, healthcare, transportation, and benefits.
• Assist consumers in accessing housing resources, vouchers, and rental assistance.
• Provide follow-up and wrap-around support to promote housing stability.
• Guide consumers through Medicaid, waiver services, and other community supports.
• Provide consultation to Navigators and staff on complex cases.
• Build relationships with housing, healthcare, government, and community partners across 13-county area.
• Identify barriers and collaborate to improve services and policy-level solutions.
• Represent The Ability Center in community meetings and coalitions.
