The Housing Medical Case Manager I at University of Louisville will provide housing-focused case management and advocacy to Ryan White-eligible clients across the KIPDA region, promoting treatment adherence and stable housing.

Responsibilities include eligibility assessments, individualized housing plans, and coordination of services with housing, health, and community partners. Strong documentation and care coordination skills are essential.

Qualifications

• Bachelor's degree or equivalent combination of education and experience is required.
• Grade 4 (Hourly) noted in minimum requirements.

Responsabilités

• Maintain an active caseload with housing-focused non-medical case management.
• Conduct eligibility, acuity, housing and needs assessments (including VI-SPDAT).
• Develop individualized housing plans and complete recertifications every six months.
• Provide end-to-end housing navigation, including applications and budgeting.
• Coordinate housing transitions and monitor progress of plans.
• Collaborate with internal staff, providers and housing partners for care linkage.
• Develop community partnerships to expand housing opportunities.
• Maintain documentation in EPIC, CareWare and required systems.
• Monitor referrals, client engagement and outcomes; provide timely follow-up.
• Participate in quality improvement and Ryan White compliance activities.
