Qualifications

• High School Diploma required.
• Demonstrated cultural awareness and respect for diverse populations.
• Experience with individuals experiencing homelessness preferred.

Responsibilities

• Assesses strengths and needs of referred people, families, and communities.
• Provides home and community behavioral health services that are practical and collaborative.
• Meets people where they are with support for self-management of health and well-being.
• Maintains documentation and participates in program evaluation and meetings.
• Meets billing productivity requirements.
